Púrpura trombocitopénica autoinmune crónica en adultos
Análisis de 80 pacientes observados durante una década
Abstract
Eighty adult patients with well documented Chronic Idiopathic Thrombocytopenic Purpura(OTP) seen at the hematology service of the Hospital Universitario San Vicente de Paúl of the city of Medellin, Colombia, during an eight-year period (1982-1990), are the subject of this report. Oral steroid therapy, as initial treatment, resulted in a 26.3% permanent remission rate. Forty patients (50%) were steroid-dependent, for this reason underwent splenectomy, of them 31 (94%) had sustained remission. On the other hand, only 2 of 15 patients that did not respond to steroids had sustained remission after splenectomy (p < 0.001). Age under 40, quality of response to steroids, and thrombocytosis following splenectomy were all good predictors of sustained permanent remission. It is concluded that adults with CITP have a sustained remission rate of 78.3% after steroid therapy and splenectomy.
