Although the discovery of a congenital interventricular septal aneurysm is often incidental, here we present the case of an adult patient with syncope and sustained monomorphic ventricular tachycardia on electrocardiogram as the only manifestation. Subsequent studies showed the septal defect, which resulted in the implantation of a bicameral cardiodesfibrillator as a method of secondary prevention.
